Soft Tissue Profile Changes Following
Treatment of Class II Division 1 Malocclusion
with Different Orthodontic Modalities

Abstract :
Objective To compare the soft tissue profile changes of Class II Division 1 patients who were treated by different orthodontic modalities.
Materials and Methods The subjects comprised 104 Thai growing patients who presented Class II Division 1 malocclusion and were treated by different orthodontic modalities. Group I: 30 patients were treated with cervical headgear. Group II: 30 patients were treated with Class II traction. Group III: 44 patients were treated as an extraction of the four first premolars. The profile changes were scrutinized from lateral cephalograms before and after treatments by means of the X-Y coordinate system. Significant differences of the profile changes within and between groups were tested by paired t-test and one-way ANOVA at 0.05 significant level, respectively.
Results In the headgear group, the upper lip moved backward and downward, the lower lip only moved downward and the chin moved forward and downward. In the Class II traction group, the upper lip moved downward, the lower lip moved forward and downward and the chin moved downward. In the extraction group, the upper and lower lips moved backward and downward whereas the chin moved forward and downward. There were significant differences among the three groups at the upper and lower lips. The headgear and extraction groups had more upper lip retrusion than that of the Class II traction group. The extraction group had lower lip retrusion, whereas the Class II traction group had lower lip protrusion.
Conclusion Different treatment modalities as well as facial growth produce different facial profile responses in Class II Division 1 growing patients.
